Challenges Kenyans Face In Accessing Healthcare And How Equity Afya Helps Out

Financial Strain

A significant portion of Kenyans shoulder healthcare costs directly, with approximately 150 billion shillings spent out-of-pocket annually. This financial burden is exacerbated by rising healthcare costs, which increased by 3.3 percent over the past year.

Although the Social Health Insurance Fund (SHIF) was introduced to alleviate such burdens, its implementation has been fraught with challenges. As of early 2025, only about 15 million people had enrolled, leaving a substantial portion of the population without coverage.

Healthcare Workforce Shortages

Kenya faces a critical shortage of healthcare professionals. The doctor-to-patient ratio stands at approximately 1:5,263, significantly below the World Health Organization’s recommended 1:1,000. This deficit is more pronounced in rural areas, where access to medical personnel is limited. Contributing factors include inadequate training facilities, poor remuneration, and the migration of healthcare workers seeking better opportunities abroad.

Infrastructural Deficiencies

Many healthcare facilities, especially in rural regions, lack essential equipment and infrastructure. A 2023 health facility census revealed that approximately 93 percent of public healthcare facilities are ill-equipped to provide primary care, and 78 percent cannot offer critical care services. Such deficiencies hinder the delivery of quality healthcare and exacerbate health disparities across the country.

Systemic and Administrative Challenges

The transition from the National Health Insurance Fund (NHIF) to the Social Health Authority (SHA) aimed to streamline healthcare financing. However, the shift has been marred by reimbursement delays, financial instability among healthcare providers, and operational inefficiencies. As of February 2025, the SHA’s implementation score had declined, indicating regression in key service delivery areas.

Corruption and Mismanagement

Corruption remains a significant impediment to healthcare delivery in Kenya. Instances of embezzlement, procurement fraud, and mismanagement have diverted essential resources away from healthcare services. Notably, the Kenya Medical Supplies Authority (KEMSA) was embroiled in a scandal involving the misappropriation of funds meant for medical supplies during the COVID-19 pandemic.

Technological Fragmentation

The lack of interoperability among healthcare information systems leads to fragmented patient data, duplication of services, and medical errors. Despite initiatives like the Kenya Health Information System (KHIS), inconsistent implementation hampers the effectiveness of digital health solutions.

Public Perception and Trust

Public confidence in the healthcare system has waned over the past decade. A 2023 report indicated a decline in public perception scores, reflecting concerns over accessibility, affordability, and quality of care. This erosion of trust can deter individuals from seeking necessary medical attention, further impacting health outcomes.

How Equity Afya is saving the ship

Equity Afya plays a pivotal role in addressing Kenya’s multifaceted healthcare challenges. With over 130 medical centers strategically located across urban, peri-urban, and rural areas, Equity Afya enhances geographical access to healthcare services, reaching underserved communities.

Furthermore, the initiative contributes to alleviating workforce shortages by engaging qualified medical professionals, many of whom are alumni of the Equity Leaders Program, thereby ensuring the delivery of quality care.
